When to use flat fees to pay doctors/ medical center?

I am wondering about the best way to pay a doctor at a medical center. Is it a flat fee?

Physicians typically bill based on the level of the exam and the procedures that were done. It's not a flat fee all you can eat buffet. It might be a $100 bill to see the doctor alone and then everything else is extra. Labwork is usually billed separately.
